With the purpose of entirely dedicating our initial meeting time to the subject of your consultation, we have made available to our clients, the initial package with the required forms in 2 formats.
Online: If it is your preference to complete the forms before your arrival, please let us know, to email you the code, so you may download, print and complete the forms to bring them completed to your first appointment.
On site/In our office: To avoid the hassle of downloading and printing the forms, if you rather fill out the forms and questionnaires directly in our office, please plan on arriving 20 minutes earlier to your initial consultation session, to have enough time to fill out the forms.
Please make note that the Spanish forms are only available on site.
